How to Apply for the CNA Certification Exam? (Oahu)

For Oahu Applicants Only:

Note: ARC Applications are taken every Thursday by Norma Lauritzen. Please call to make an appointment.

For your Application to be accepted for processing, you must comply with the following requirements.

1. Complete American Red Cross Examination Application Form.

2. Make a copy of the following:

a) Certification of Nurse Aide Training Program (NATP) Completion issued by HT&CC, Inc. (Must bear

100 hours information.)

b) State ID or Drivers License or any ID that has your Photo. (E.g. Passport)

Note: You must present originals to the Testing site on the scheduled day of your exam.

c) Social Security Card

3. Buy a money order/cashiers check payable to to the American Red Cross in the amount of $250.00.

4. Pay processing fee to HT&CC, Inc. ($10.00)

5. If you have a criminal record, attach a copy of your criminal abstract to your application form. You may also attach any additional documentation explaining your criminal abstract.

6. Please bring in the following requirements to:

Important Reminder: American Red Cross Application Forms and Requirements must be submitted 2 days prior to the deadline. You may obtain dates of deadlines from Norma. In addition, ARC will send you a confirmation letter, please read this letter carefully. Call ARC at (808)734-2101 if you do not receive your confirmation letter 3 days prior to your scheduled exam date. HT&CC, Inc. does not have the ability to negotiate any changes to your test date with American Red Cross. You need to call them directly.
